At some point in the last few weeks, you will have received a package of paper forms with a cover sheet with instructions for completing online consent forms. Both the paper and the online forms were to have been completed and returned by today (Tuesday, October 13th).
You are receiving this notification because our online platform is down which means you will not be able to complete the online forms.
Please return the paper forms (Verification & Emergency Release) and retain the cover sheet for your future reference. We will contact school families once the online consent form platform is accessible.
The online consent forms ask for permission for the following:
- Consent to send commercial electronic messages (CASL) – permission to send newsletters, community announcements, performance/sport notices, event invitations, surveys, etc. by email;
- Media Release
- Student Personal Information
- Walking Permission – neighbourhood walking trip or activity related to their studies. A separate field trip permission form for field trips that require vehicle transportation or are longer in duration will be sent home.
- 0365 Permission – student’s agreement to abide by the District Technologies & Information Systems Student Acceptable Use Agreement (please see page 16 of your student’s planner for this agreement).
While our online consent form platform remains down, we will operate with the understanding that:
- we MAY send you emails (item#1),
- that your student has agreed to O365 Permission (item #5)
- that your student may participate in neighbourhood walking trips (item #4).
We will contact you directly should we require permission for items #2 or #3.
